He launched Barstool Sports in 2003 as a free print publication in the Boston area before transforming it into a major online platform with a devoted following through irreverent humor, gambling commentary, and viral content. Portnoy has also gained widespread recognition for his "One Bite" pizza review series, in which he travels to pizzerias and rates slices on a simple scale, amassing millions of views across social media platforms.

Under his leadership, Barstool Sports expanded significantly, partnering with Penn Entertainment in 2020 to enter the sports betting industry. The relationship later soured amid public disputes and ended in 2023 when Penn divested its ownership and Portnoy repurchased 100% of Barstool Sports. Portnoy is the son of Michael Portnoy, a lawyer, and Linda Portnoy, a math teacher at Swampscott High School.
